In Mouth of Madness, the player has to execute the pink dress-wearing Smiley with a Meat Cleaver, decapitating his head, and take the head to the guard room.
Kill a Smiley wielding a Meat Cleaver and pick it up. Progress to the roof section on the tower, perform any execution level on the pink dress-wearing Smiley, take his Head and show it to the Director in the Guard Room.
