Mouth of Madness is the 11th scene featured in Manhunt.
Plot Summary[]
James Earl Cash is taken back to his old Darkwoods Penitentiary. Cash has to execute a Smiley and take the dead body to the guard room.
Scene[]
Introduction[]
The scene starts with a male voiceover and an overview, showing more about the Darkwoods Penitentiary. It has more than a thousand inmates who came from all over the United States of America. The prison's modern facilities include spacious cells for the inmates and a well staffed hospital equipped for emergencies as well as routine health checks. In ROW 1, the electric chair room is where all death sentences are carried out. The introduction shows the abandoned penitentiary, showing crazed inmates with smiley masks. The scene moves to where the Cerberus are taking Cash with a black bag on his head to a spot near the cells, where the Cerberus Leader drops him, taking the bag off and leaving Cash alone.
The scene moves to the director overviewing the asylum. The director says The Smileys have taken over the penitentiary, and that the only way to deal with them is to execute or shoot them. The director wants Cash to execute the dressed Smiley patrolling the tower walkway and bring the body to the Guard Room, after which the camera moves to the Guard Room.
A ward is then shown, with a hunter in front of the door, and Cash behind it. The director tells Cash to bait the hunter to open the mesh door, as killing him will get the director to open the next set of doors.
Ward[]
Cash picks up a Green Class Weapon – a Wire. Cash finds a switch and a male locked in an electric chair. Cash pulls the switch down, makes noise from the electric chair, and baits the hunter. The hunter opens the mesh door to switch off the electric chair, which is when Cash executes him. After the hunter is executed, the director opens the next set of doors.
Park[]
There are more Smileys guarding the door to the greenhouses; if they spot Cash, they will lock the door to the greenhouses, and then reopen it when they lose Cash. Cash executes the three Smileys, the last of which is positioned on a guard tower and equipped with a Revolver Six Shooter, Cash baits him downstairs, and when the Smiley comes, Cash gets behind and executes him, keeping the door to the greenhouses open.
Greenhouses[]
Cash kills the hunters with firearms and melee weapons, and one hunter blocking a path to the guard tower. A hunter hides in a dark place, waiting to scare the player, the player kills the hunter. Cash finds out the door to the guard tower is bricked for the Smileys to hide, he finds a gasoline can in the second greenhouse, places it in front of the bricked door, and shoots the gas. The gasoline blows up the bricked door, gaining access to the tower.
Guard Tower[]
Cash goes upstairs, with one hunter guarding the tower. Reaching upstairs, he pulls down a lever opening the second door leading to the main asylum.
Main Asylum[]
Cash walks on finding an exit to the roof, while killing many Smileys.
Roof[]
Cash kills many Smileys guarding the dress-wearing hunter. Cash succeed and decapitates the hunter's head or kills the hunter, holding the head/body to the guard room. The director tells Cash to drop the weapons and the body to continue on to the next scene.

- The Skinz were originally supposed to be in this level. Shown in an official pre-release screenshot.[1]
- According to this scene's
.GXT
file, Cash was supposed to kill a hunter wearing a monkey costume, bring the body to the Guard Room, instructed to drop his weapons and wear the monkey costume. This may be the reason why the director says "Monkey see, monkey kill!" in the scene Doing Time, and the reward for finishing Doing Time is unlocking Monkey See, Monkey Die!. - "Mouth of Madness" might be a reference to the following:
- The name of a horror film directed by John Carpenter, whose music appears to have a great influence on Manhunt's soundtracks.
- The Smileys, a gang of insane inmates who mostly wear a smiley face as a mask or a face painting.
- The gaping hole on the floor of this level's Guard Room, which leads into the prison itself as Cash descends further into the mouth of madness.
